Application Process

This application form can be completed by the applicant themselves, or by their Personal Assistant (PA). If you are a PA who cares for more than one individual you will need to apply separately for each person you assist, because complimentary PA tickets are assigned to the applicant, not the PA.

This form contains 3 Sections

Sections A and Section B will need to be filled out for all applications. We ask for this information so that we can provide the best possible service for you when booking tickets.

Section C only needs to be completed for those who require a Personal Assistant to attend our venue with them; this section of the form requires you to provide proof of eligibility.

Please note that providing information is not a guarantee that accessible seats will be available at all performances, although we will endeavour to always accommodate requests.
