Make Music: Young People Acceptance Page 23/24

To accept your place on Make Music, Young People for 23/24 please follow the steps below by Monday 10th July.

If you need any help you can drop us a line on

1. Read the info:

At the bottom of this page are some links to pdfs.  These include all the details for schedules and fees for all the different programmes, and our Fair & Square “code of conduct” style document to help everyone get the most out of their time with us.

2. Fill in the Acceptance Form:

Click here to complete the MMYP Acceptance FormYou will need your UNIQUE CODE from your offer email.

You should do one form for each young person in your care that’s joining the programme.  This form gives us more details that we need to hold, and also includes consent questions, where we ask for your permission for your child to take part in the programme, for data usage, as well as asking you to confirm you understand our various terms and conditions.  It must be completed by the parent/carer or a student over the age of 18.

3. Apply for Financial Assistance:

There are two bursaries available:

  • Centre for Advanced Training Students ONLY

Music and Dance Scheme (MDS)

You can either complete the word version or excel versions linked below – you don’t need to complete both!  If you have questions about the form you can either find the answers in here, or by emailing us your query – we know the form is complex and we are happy to help.

MDS form – word version  You can print this and complete it by hand, or complete it on your computer and email it to us at with scans of your evidence.

MDS form – excel version  New for this year – this makes our lives a little easier at our end, but probably best only for those of you that have completed this form many times!  Email it to us at with scans of your evidence.

If you prefer to post your documents you can do so to: Hannah Fumoleau, Sage Gateshead, St Mary’s Square, Gateshead NE8 2JR

Final note: MDS is an external bursary administered by us in connection with Department for Education. We will therefore be sharing data with DfE – you can read their full privacy notice here
